脉冲染料激光治疗婴幼儿体表溃烂型血管瘤疗效研究

摘 要:目的:观察波长为585 nm脉冲染料激光治疗婴幼儿体表溃烂型血管瘤的临床疗效。方法:采用波长为585 nm的脉冲染料激光,对76例(男26例,女50例)体表溃烂型血管瘤(平均瘤体表面积22.1cm2)患者进行照射治疗,激光能量为5.8 J/cm2~8 J/cm2(平均6.6 J/cm2),间隔4周治疗一次,治疗二次后瘤体无消退者予口服类固醇辅助治疗,类固醇辅助治疗4周无效者再联合干扰素治疗,直至创面愈合和瘤体消失。结果:68例患儿在治疗二次后创面完全愈合,瘤体明显缩小,经一至八次(平均四次)激光治疗后瘤体完全消退,8例患儿辅以类固醇治疗,6例治愈,2例无效后予干扰素联合治疗后瘤体消退,总治愈率100 %;经过平均12个月随访,所有患儿无溃疡复发和血管瘤再发。结论:脉冲染料激光治疗婴幼儿体表溃烂型血管瘤,具有良好疗效,是一种行之有效的治疗方法。


Efficacy of Pulse Dye Laser Therapy for the Treatment of Ulcerated Haemangiomas in Infant

Abstract:Objective:To investigate the efficacy of pulse dye laser therapy for the treatment of ulcerated haemangiomas in infant. Methods:Seventy-six children (50 girls, 26 boys) with ulcerated haemangiomas, from our vascular malformation clinic were enrolled in a prospective pulse dye laser treatment protocol from April of 1999 to March of 2006. The mean size of the ulcerated haemangiomas requiring treatment was 22.1 cm2. Lesions were treated with minimal debridement and the Cynosure pulse dye laser with a mean energy of 6.6 J/cm2(5.8 J/cm2~8 J/cm2)per treatment. Lesions were treated in a sequential pattern at 4 week intervals until cutaneous healing or involution of the haemangioma occurred. Results:Sixty-eight of the 76 patients (89.5 %) responded to laser therapy alone with a mean number of 4.0(1~8)treatments. Eight patients with very large haemangiomas required oral steroids (2 mg/kg·d-1~3 mg/kg·d-1) in combination with the pulse dye laser. After failing to improve on steroid therapy, two patients required the addition of interferon to their treatment protocol. The rate of clinical efficiency was 100 %.The mean follow up time is 12 months with no sign of recurrent ulceration or regrowth of the haemangiomas in our study population. Conclusion:Pulse dye laser therapy is a reasonably effective means of resolving the untoward complication of ulceration of haemangiomas.
